Terms of Membership
Membership to the HCD Health Panel is voluntary and participation in the panel and all associated surveys is subject to the terms and conditions set forth in this member agreement. Please review this member agreement thoroughly to ensure that you understand and comply with all terms and conditions of your membership.

HCD Research reserves the right to add, remove, or modify this member agreement in part or in whole,
at any time and at our sole discretion.
Continued use of this website after changes or modifications to the Terms of Membership
constitutes acceptance of the terms and conditions outlined below.
If you do not agree to the membership terms and conditions or any proceeding modifications,
or are otherwise dissatisfied with the services provided, you may at any time terminate your membership to
the HCD Health Panel.
By participating in the HCD Health Panel, you are stating that you agree to the following terms:
Eligibility
Membership to the HCD Health Panel is open to healthcare professionals practicing in the United States.
If you reside outside of the United States, you are not eligible for membership.
Honesty and Integrity
In order to provide our clients with quality results to their studies,
it is important that the integrity of the data is upheld. Therefore, the honesty
of our panel members is very important. You as a member of the HCD Health Panel agree to
provide honest and accurate data during the registration process as well as all associated surveys.
If you provide inaccurate or false information, HCD Research reserves the right to terminate your
membership.
Non-Transferability
Membership to the HCD Health Panel is limited to one person with a unique email address.
Members may not register, take surveys, or otherwise perform panel functions on behalf of other individuals.
Zero Tolerance for Fraudulent Activity
Members agree not to abuse the benefits and privileges as HCD Health Panel members. Such abuses included, but are not
limited to, attempts to create multiple accounts and attempts at participation in surveys by tampering with surveys or links.
Number of Memberships
One membership will be issued per person. Various verification methods are used to ensure that members do not register more than one time for the HCD Health Panel.
If we find any fraudulent activity of a panel member, the membership(s) will be terminated.
Failure to Comply
Failure to comply with all membership rules or any activity that has been detected and found to be fraudulent or abusive
may result in the forfeiture of all earned incentives as well as the termination of membership to the HCD Health Panel.
HCD Research reserves the right to determine such cases, and any decision made by HCD Research is final.
